The Deformed Revenant has a chance to spawn when a Zombie-type mob is killed during Revenant Horror Tier IV quests. Its spawning is marked by explosion sounds and particles.

Deformed Revenants will seek out and attack the player who summoned it. They have a short player detection range and are slower than the weaker Revenant Champion.
It deals 4,000 damage on hit, making it one of the deadlier enemies; it is considered by some to be harder than Tier IVRevenant Horrors.
